NHS App

If you're a patient at our practice you can download and use the NHS App, a simple and secure way to access a range of NHS services on your smartphone or tablet.

You can use the NHS App to check your symptoms and get instant advice, book appointments, order repeat prescriptions, view your GP medical record and more.

If you already use other online access providers you can continue to use these and you can use the NHS App as well. For more information go to www.nhs.uk

SystmOnline

The practice will provide online access for:

  • appointment booking, ordering repeat medication, completing questionnaires and viewing your summary care record (basic information showing medication, allergies and sensitivities)
  • access to coded and clinical data from within the record. The record must be reviewed by a GP prior to access being granted.
    Photo ID and proof of address is required, which can be done using the NHS App.

    Church Lane Surgery will provide online services to patients as detailed below:

Access to appointments, repeat prescriptions and summary care record. Photo ID verification is required.

Further access to your detailed coded medical record pre 1.4.2019 and full clinical record thereafter can be requested. Photo ID verification and additional authorisation by a GP is required (allow 20 working days).

Access to appointments, repeat prescriptions and summary care record. Photo ID verification of both parties is required.

11 - 15 years The practice does not offer online services to patients aged between 11 and 15 years unless there are exceptional circumstances.

11 -15 years

(Proxy Access in exceptional circumstances)

The practice recognises that in exceptional circumstances it may be a requirement for a parent/carer to have proxy access to a child’s record after the age of 11 years. Each case is individually assessed by a GP who will decide if proxy access is to be authorised.

If proxy access is granted it will be for basic online access only i.e. access to appointments and repeat prescriptions.

On the young person’s 16th birthday the exceptional proxy access will be reviewed by the practice.

A young person may decide at any point after their 16th birthday to request access themselves.

0 - 10 years

Access to appointments, repeat prescriptions and summary care record. Photo ID verification is required for the parent/ guardian and a copy of the childs birth certificate.

Identity verification is required before online service applications can be processed. At the point of application the practice will verify identity using one of the methods detailed below:

Documentation - One form of photo ID and one proof of address

  • Either of the following: Passport or Photo Driving Licence
  • One official letter bearing your name and address dated within the last three months from any of the following: your bank, building society, local council, utility company or landline telephone provider
  • NHS App approval. The practice will be notified of confirmation of ID
