Online Services


If you're a patient at our practice you can download and use the NHS App, a simple and secure way to access a range of NHS services on your smartphone or tablet. 

You can use the NHS App to check your symptoms and get instant advice, book appointments, order repeat prescriptions, view your GP medical record and more.

If you already use other online access providers you can continue to use these and you can use the NHS App as well. For more information go to


The practice will provide two levels of online access:

  • Standard access - Access to appointment booking, ordering repeat medication, completing questionnaires and viewing your summary care record.
  • Enhanced access - Access to coded and/or clinical data from within the record. Please note that you must have standard access to be eligible for this service. The practice requires the patient to request enhanced access and the record must be reviewed by a GP prior to access being granted. 
    Photo ID and proof of address is required on application.

    NB this service is currently suspended as we are restricting patients attending the practice due to COVID-19

Church Lane Surgery will provide online services to patients as detailed below:

Age of Patient Services Offered
16 years and over

Standard Online Access - access to appointments, repeat prescriptions and summary care record, as soon as the application has been processed and identification verified. (Allow 5 working days).

Enhanced Online Access – access as above but also included is access to detailed coded medical record or full clinical record, once the application has been processed, identification verified and the additional authorisation process has been completed by a GP (Allow 20 working days).
11 - 15 years The practice does not offer online services to patients aged between 11 and 15 years unless there are exceptional circumstances.

(Proxy Access in exceptional circumstances)

The practice recognises that in exceptional circumstances it may be a requirement for a parent/carer to have proxy access to a child’s record after the age of 11 years. Each case will be assessed on an individual basis by a GP who will decide if proxy access is to be authorised. 

If proxy access is granted it will be for standard online access only - access to appointments and repeat prescriptions, as soon as the application has been processed and identification verified. 

On the young person’s 16th birthday the clinical system will switch off all the remaining proxy access except where the young person is competent and has given explicit consent to the access.  A young person may decide at a point, once they are mature enough to act autonomously, to request access themselves.

0 - 10 years

Standard Online Access - access to appointments, repeat prescriptions and summary care record as soon as the application has been processed and identification verified. 

Identity verification is required before online service applications can be processed. At the point of application the practice will verify identity using one of the methods detailed below:

Documentation - One form of photo ID and one proof of address

  • Either of the following: Passport or Photo Driving Licence
  • One official letter bearing your name and address dated within the last three months from any of the following: your bank, building society, local council, utility company or landline telephone provider

The practice requires 20 working days to process enhanced access (access to detailed medical records).  This is to allow for the GP data checks to be completed.

Note: The practice requires that the patient / proxy provide proof of identity on collection of user credentials. User credentials will not be released without this.
