New Patient Registration

Thank you for choosing to register with Church Lane Surgery.

Here you will find a Primary Care Team including Doctors, Practice Nurses and other healthcare professionals. Please read this introduction carefully and return all completed forms as soon as possible.

You do not need to know your NHS number to use NHS services, but it can be useful to have it

Online Registration Form

You will need to complete the online registration form to register with our practice. This form will ask questions including details of your previous GP surgery and basic health and medical information. This form will ask questions including details of your previous GP surgery and basic health and medical information. If English and you need help completing the form, please go to reception and they will be able to help you complete the form, with the help of an interpreter if required.

If you don’t have a fixed address or are homeless, please discuss with reception as you can still register and use the surgery address as an address (care of). 

You may be asked to provide a copy of your ID for a proof of an address. For example, valid ID could include a photograph of your passport or driving licence. This is to check that you live in the local catchment area.

The paper registration form is available from reception at the practice or download it here (PDF, 229KB).  When you come in to collect the registration form the receptionist will also give you a welcome pack which will include; an electronic patient record and information sharing form, a summary care record form and a new patient medical questionnaire form.

New Patient Registrations

The practice is computerised. Patients who register with the practice will have their personal and clinical details recorded on the computer system, Systmone. All new patients will be asked to complete a simple questionnaire about their health, past medical history, family history, address and telephone details. We ask that patients keep the practice informed of any change in these details as they occur.

All patients will be allocated a named GP who is responsible for the patient's overall care. Although you will have a named GP, you can still make appointments with any of our doctors. If you wish you can also ask to change your named GP and we aim to accommodate this request wherever possible.

Everyone in the UK has the right to free care from a GP. If you don’t have proof of ID or address, you can still register. Immigration status or nationality don’t matter - reception won’t ask for immigration documents and won’t share your information with the Home Office unless serious crime is involved.

New Patient Medical Health Check

All new patients are asked to make an appointment with the Practice Nurse to carry out a basic health check. This will take approximately 20 minutes. Please bring to this appointment your completed medical questionnaire and a sample of your urine (specimen bottle provided). At this appointment your height, weight, blood pressure etc. will be checked.

This basic medical information is important to your new doctor as until we receive your medical records from your previous surgery we know nothing about you; so it is essential to have your help with this information to ensure best possible care.

The new patient check is available for all patients five years of age and over. Children under five will be welcomed at our regular children's clinic - please complete the additional form for under-five's.

Your First Appointment with the Doctor

If you have on-going medical problems which will need to be discussed with your new doctor, please ask the receptionist to book you a double appointment for the first time that you see the doctor. This is very important for both you and your doctor.

Repeat Medications

If you are on medications from your previous doctor please make an appointment to see or speak to a doctor here at least two weeks before your current supply runs out.

Online Services

You can order your repeat prescription(s), book appointments, change your address, complete questionnaires online, view your summary care record and access your detailed coded data within your medical record. If you would like to use this service, please complete and return to reception the relevant online booking application form which is available from reception or via the Services / Online Services section on the website or Online Service application forms here.

Please bring with you photo ID (e.g. Passport or Driving Licence) and proof of residency. When the form has been processed you will receive a user name and password so that you can book your appointments or request your repeat prescription by computer or through your smart phone app.

LGBT+ Patients 

All our staff at the practice are working to improve their understanding of the issues facing our LGBTQ+ patients and positively support them in accessing the care they need. We support everyone regardless of gender identity or sexual orientation and strive to ensure all our patients have an empathic and quality experience.

We recognise that the NHS registration form doesn’t have a place to put preferred pronouns or name you would preferred to be known as if this is different from your legal name. If you would like your preferred pronouns changing or your ‘known as’ name adding to your record, please contact reception who will be happy to help.
