In accordance with the Data Protection Act 1998 and GDPR 2018 patients may request copies of all or part of their medical records. Requests can be made in writing, or verbally to the practice. There is no charge for this unless requests are excessive. It is known as a Subject Access Request (SAR)
Through the online services, you can view your summary care record or access your medical record online. To access this facility you will need to complete an application form and provide photo identification (Passport or Driving Licence) and proof of residency (utility bill or bank statement). Your GP will review your record and has the right to redact information relating to 3rd parties or sensitive information which may cause harm.
In accordance with Access to Healthcare Records Act 1990 the practice will provide copies of your medical record or a report to 3rd parties for purposes such as setting up insurance, police reports, and solicitors reports. No information will be released without the patients consent unless we are legally obliged to do so. There is an administration fee for these requests.
